By signing this document, the Client authorises Artemide s.r.l. to send to the partner clinic, based in Istanbul, (‘the Clinic’) the photographic documentation showing the situation as of today’s date of the area involved in the hair transplant.

Artemide s.r.l. specifies that:

– the transmission of the photographs will be carried out directly by the Client by uploading them onto the portal made available by Artemide s.r.l.

– The signing of this form does not in any way bind the Client to sign a binding contract either with Artemide s.r.l. or with the Clinic, but is only functional for the pre-assessment of the clinical case that will be carried out by and under the direct responsibility of the Clinic.

– Artemide s.r.l. will share with the Client the result of the clinical case pre-assessment issued by the Clinic, consisting only in the judgement of feasibility or non-feasibility of the health treatment; further health information will not be shared by the Clinic with Artemide s.r.l.

– In case of a positive judgement of feasibility of the health treatment, the Client shall be free to sign or not to sign a binding contract to proceed with the therapy proposed by the Clinic.

– Should the Client, at his own unquestionable discretion, decide not to carry out the proposed therapeutic treatment, notwithstanding the Clinic’s feasibility assessment, nothing shall be owed by him either to Artemide s.r.l. for the information and intermediation activities carried out, or to the Clinic for the pre-assessment of the clinical case.

Obligations of Amantide:

The Organisation undertakes to maintain the utmost confidentiality on all data and information provided by the Client. This data will not be disclosed to third parties, nor will it be used for purposes other than those strictly related to the proposed course, except as necessary for the execution of the contract or the Client’s express authorisation or as required by law.

Obligations of the Client:

The Client undertakes not to use the information received from Amantide for purposes other than those of the proposed route. In particular, the Client acknowledges Amantide’s role in the referral of partners and for this reason undertakes not to directly contact partners or other parties involved in the proposed route without Amantide’s prior written consent for a period of 3 years from the date of the first Amantide contact.

In the event of a breach of this undertaking, the Client is obliged to indemnify Amantide for the loss of earnings, all exceptions removed.
Any use of the information outside the agreements set out in this contract will be considered a breach thereof.
